This Leave Form is for members of VP Staff but can be utilized by other staff members at the direction of their supervisors. Understand your learners’ experiences and assess outcomes across campus in real time through surveys, rubrics, and polls. Anthology Baseline supports the full data collection cycle – from planning to action – so you can make the most of your assessment efforts and positively impact student success.
All DOSA members can log-on and use Anthology/Baseline for assessment projects in your area by contacting the Planning and Assessment Team (PAT) Committee chair Rebekkah Wall (RebekkahWall@MissouriState.edu) for access to missouristate.campuslabs.com.

Since MSU has an institutional membership with NASPA (Student Affairs Administrators in Higher Education), staff can become a member at a reduced rate, pending supervisor approval, of course. Please contact PeggyJones@missouristate.edu if you need assistance with your individual membership with NASPA.
Benefits to joining NASPA include:

Student Affairs has a webpage dedicated to Professional Conference Travel. All University travel across all funding sources requires supervisory and funding administrator approval prior to confirming travel arrangements. The Form for Pre-Travel Authorization must be completed prior to confirming travel arrangements when the travel is overnight or out of state. To the extent that funds are available, travel funds will be provided to support the participation of full-time staff members in order to attend professional conferences and to present.
A sampling of conferences and development opportunities include:
- Members receive significantly discounted rates for all NASPA events, including the 2025 NASPA Annual Conference taking place in New Orleans.
- NASPA IV-West Conference 2024 conference registration
- The 21st Public Affairs Conference is Sept. 24–26, 2024. Free and open to the public, this year's conference offers a mix of in-person speakers and live virtual panels.
- The Collaborative Diversity Conference held each spring.

- Magers has a full and part-time medical staff of 14 physicians and one nurse practitioner. Specialties covered include family medicine, internal medicine, psychology, nutrition and diabetic counseling, gynecology, psychiatry, orthopedics, and radiology. All of the family and internal medicine physicians are board certified. Employees whose normal work location is on the Springfield campus are not required to use sick leave when they have a medical appointment or seek medical treatment at Magers.
- The Employee Wellness Program strives to provide meaningful programming that addresses all components of the human being namely social, emotional, spiritual, environmental, financial, intellectual, physical and occupational in a manner that is respectful of the diversity of the community at large.
- Mercy's online Employee Assistance Website, accessible with the username and password MSU, features a wide array of legal resources, including articles, legal forms, and a glossary of terms, covering topics such as bill of sales and limited power of attorney forms.
- The Employee Assistance Program (EAP) offers confidential assistance to employees and their household members, available 24/7. This service is provided at no cost to both full-time and part-time employees.
- Mercy's EAP partners with a third-party vendor to offer free legal consultations. To request legal assistance, call Mercy EAP at 800-413-8008, selecting option #2.
- Legal Assist provides support for various legal matters, excluding disputes with employers or business issues. Common issues addressed include divorce, criminal matters, estate planning, real estate, bankruptcy, and more. Services include telephonic consultations with participating attorneys and connections to local lawyers for in-person consultations, often at discounted rates. (Typically, Free half-hour consultation, and, in most instances, discounted hourly fees by 25% if additional assistance is required.)
- The Bear Pantry offers a food source for struggling students, faculty, and staff.
- Membership at the Foster Recreation Center is open to all Missouri State students, faculty, staff, active alumni, University affiliates and household members at varying rates.
- Outdoor Adventures supports your outdoor pursuits by providing quality equipment rental at reasonable prices.
- Shared leave enables employees to support co-workers during leaves, ensuring uninterrupted income
through donated vacation time, subject to eligibility and committee approval.
- Incapacity due to pregnancy, prenatal medical care or child birth
- To care for the employee’s child after birth, or placement for adoption or foster
care
- 80 hours outlined under parental leave (paragraph 7.19.4-D)
- To care for the employee’s spouse, sponsored dependent, son, daughter or parent, who has a serious health condition
- For a serious health condition that makes the employee unable to perform the employee’s job
- Bone marrow and organ donations (employee donating)

- Human Resources at Missouri State lists discounts for businesses with physical locations within a half day's drive of Springfield, Missouri. Posting discounts does not indicate university support of the business.
- Faculty/Staff of Missouri State can receive various discounts to tickets at certain Missouri State University sporting and theater events.

- In 1964, Missouri State University opened a preschool laboratory for practicum experiences in child and family development and early childhood education. This evolved into the Child Development Center in 1999, combining infant, toddler, and preschool programs into a single facility at 609 E. Cherry. Part of the College of Education, it offers various activity areas, classrooms, and support spaces, serving up to 88 children from 6 weeks to 6 years old. While there is a waiting list, employees get priority enrollment (following sibling placement) and receive discounted rates. Enjoy peace of mind with this invaluable resource, staffed by qualified teachers and supervised practicum students.
